Preschool EI Forms
The Department of Education, Bureau of Special Education preschool early intervention program has a variety of standardized forms that meet necessary state and federal requirements. These forms are designed to ensure that preschool children with disabilities receive a free, appropriate, public education. Infant/Toddler EI Forms
The Pennsylvania Department of Public Welfare, Office of Child Development has state forms for the Multidisciplinary Evaluation/Individualized Family Service Plan (MDE/IFSP) and the Parents Rights Agreement. These forms ensure that local programs are meeting all the state and federal requirements for the infant/toddler early intervention program.
